Here is how it can help you achieve all the new objectives in the prevention package:

Objective 1: Improve patient outcomes and improve efficiency of care after hip fractures through compliance with core standards.


How Cellma can help with compliance of core standards:

1. All patients with hip fracture should be admitted to an acute orthopaedic ward within 4 hours of presentation.

  • Fully Customisable Clinic, In-Patient, A&E and telephone assessments
    • Highlighting risk factors as well as predisposing factors (family history, past/current medical history etc...).

2. All patients with hip fracture who are medically fit should have surgery within 48 hours of admission, and during normal working hours .

  • Automated referrals by letter, email or intranet between wards, hospitals and trusts.


3. All patients with hip fracture should be assessed and cared for with a view to minimising their risk of developing a pressure ulcer.

  • Ward management tools with patient status (risk factors highlighted).


4. All patients presenting with a fragility fracture should be managed on an orthopaedic ward with routine access to acute ortho-geriatric medical support from the time of admission.

  • Patient data can be accessed by different services in different locations in real time. 
    • i.e. radiology reports/imaging can be viewed by consultant from office. 


5. All patients presenting with fragility fracture should be assessed to determine their need for anti-resorptive therapy to prevent future osteoporotic fractures.

  • Our assessments will highlight those you are in need of anti-resorptive therapy.
  • With patient and service dashboards track patient’s reaction to specific therapies and the effectiveness of such therapies within a service. 


6. All patients presenting with a fragility fracture following a fall should be offered multi-disciplinary assessment and intervention to prevent future falls

  • Software will highlight interventions that conflict with patient history allowing the optimal interventions to occur. 

 

Objective 2: Respond to a first fracture and prevent the second – through fracture liaison services in acute and primary care settings.

  • Once Cellma has helped you respond to the first fracture it will help you set up an efficient fracture liaison service (FLS) that can:
    • Highlight new fracture patients
    • Highlight patients at high risk of second/subsequent fractures
    • Incorporate BMD and clinical factors into interventions
  • The above information can then be used to automatically generate letters which can be used to liaise with GPs, PCTs and other falls services by email, post or intranet.  

 

Objective 3: Early intervention to restore independence and reduce future injuries

  • Cellma can help you establish an efficient falls care pathway as guided by the DOH.
    • The entire pathway can access live patient data from anywhere in the trust and even on the move, all in the secure N3 environment.
    • The falls service can triage and assess patients in multiple settings (home/minor injury units etc..) using Cellma.
      • Eliminating duplication, retrospective data entry and paste wastage.
    • Quality of service can be tracked through dashboards. 

 

Objective 4: Prevent frailty, promote bone health and reduce accidents- through encouraging physical activity and healthy lifestyle, and reducing unnecessary environmental hazards.

  • Relevant third parties can report into Cellma the environment hazards within patient residences.
