Chronic Heart Failure

Around 900,000 people in the UK today suffer from heart failure. An almost equal number have damaged hearts. In both cases severity increases with age. The risk of heart failure is higher in men than it is in women in all age groups. However, there are more women than men with heart failure due to population demographics in the UK.

There is no single diagnostic test for heart failure, and diagnosis relies on clinical judgements based on a combination of history, physical examination and appropriate investigations. It has been determined that the most common causes of heart failure in the UK are coronary artery disease and a history of hypertension.

The NICE chronic heart failure guideline recommends the following:

  • Patients with heart failure be given specialist advice in the following situations:
  1. Heart failure due to valve disease, diastolic dysfunction or any other cause except LV systolic function.
  2. Angina, atrial fibrillation or other symptomatic arrhythmia.
  3. Women who are planning pregnancy or are pregnant.
  4. Severe heart failure.
  5. Heart failure that does not respond to treatment.
  6. Heart failure that can no longer be managed effectively in a home setting.

Diagnostic Recommendations include taking a careful and detailed history and performing clinical examinations. These should be combined with appropriate tests and investigations to confirm or deny the presence of heart failure and make a complete diagnosis.
